What Can a Professional Care Manager do for Seniors?
 
I.                     Introduction- My background and how I came to this profession.
 
II.                  What is a Professional Geriatric Care Manager (PGCM)?
a.       A  Professional Geriatric Care Manager is a health and human services specialist who is guide, advocate, and resource for families caring for older relatives.
b.      A PGCM have extensive knowledge about the costs, quality, and availability of resources in their communities.
 
III.                What can a Care manager do for you?
 
IV.                What is the focus of my practice? Discussion and examples
a.      Conduct home assessments to identify problems and provide solutions
b.      Screen, arrange and monitor in home help or other services
c.       Advocate for the client and navigate the complex healthcare system
d.      Assist with moving an older person to or from a Retirement Complex, Assisted living or Nursing Home
e.      Act as a liaison to families at a distance
f.        Provide medication management and education
g.      Disaster planning and evacuation

Speaker's Bio:
Certified Care Manager, Cheryl Jordan graduated from the University of Maryland in 1971 with a Bachelor of Science degree In Nursing. She worked 19 years for the Veteran’s Administration as an RN. She earned a Master’s Degree in Administration in 1987 from Central Michigan University and moved on to Director of Nursing in Long Term Care for 17 years. Working with seniors and distressed families inspired her to begin a new career as a Geriatric Care Manager. In this position she is an advocate for seniors and their families assisting them with the challenges of aging.

Location: Florida Hospital Flagler on Rt 100 & 60 Memorial Medical Parkway, Palm Coast, Fl 32164. Go the the Main Hospital South Entrance, in the Lind Educational Center.

Cost: $15.00 payable at the door. Please note that persons making reservations and not canceling by noon, Monday, September 10th, will be invoiced by the EPCFC for the program cost.
